Fatigue

The most universal side effect. Usually mild to moderate, lasting 3โ€“7 days after each cycle, occasionally lingering longer. Cumulative fatigue is uncommon. Adequate rest, hydration, and light exercise help.

Dry mouth (Lu-177 PSMA therapy)

Salivary glands also express PSMA at low levels and pick up some medicine. Usually mild to moderate, persistent in a minority. Management: sugar-free gum/candy, hydration, humidifier, saliva substitutes, careful dental care.

Nausea

More common with Lu-177 DOTATATE therapy (often from the amino acid infusion). Usually well controlled with pre-medication anti-emetics. Severe nausea is uncommon.

Mild blood count changes

Bone marrow is exposed to some radiation. Hemoglobin, platelets, and white cells often dip 2โ€“4 weeks post-cycle. Most dips are mild and recover before the next cycle. Pre-cycle blood tests catch this.

Common side effects (5โ€“20%)

  • Mild abdominal discomfort โ€” particularly with NETs treatment
  • Decreased appetite โ€” usually transient
  • Constipation or diarrhea โ€” manageable with diet and OTC medications
  • Back or bone pain โ€” sometimes due to tumor flare (actually a sign of response)
  • Headache โ€” usually mild
  • Mild allergic reactions โ€” usually responsive to anti-histamines
  • Worsened carcinoid symptoms (Lu-177 DOTATATE therapy) โ€” temporary flushing or diarrhea after early cycles

For most patients, Lutetium therapy is much easier to tolerate than chemotherapy:

  • Better preserved blood counts in most cases
  • No long-term peripheral neuropathy
