The standard schedules
When fewer cycles may be appropriate
- Excellent early response โ significant tumor marker drop or imaging response after 2โ3 cycles. Some teams shorten the course if the disease is in deep response and toxicity is rising.
- Significant side effects โ persistent low blood counts, kidney function decline, or other treatment-related issues.
- Disease progression on therapy โ if the cancer continues to grow despite treatment, additional cycles are unlikely to help and a different approach is needed.
- Patient choice โ some patients decide they've had enough, and that's a valid choice.
When additional cycles may be considered
- Re-treatment after a meaningful response duration โ some patients who completed an initial course and later progressed are eligible for re-treatment with 2โ4 additional cycles years later.
- Compassionate-use protocols โ for patients with continued response but at the end of the standard 4 or 6 cycle protocol.
- Trial protocols โ some trials test extended treatment beyond standard schedules.
Frequently asked questions
If I respond well to 3 cycles, should I stop or continue?
Most teams complete the planned course (4 or 6) even with early response, because the disease can return faster if treatment is cut short. But this is a clinical judgment โ discuss with your oncologist.
Can I do more than 6 cycles of Lu-177 PSMA therapy?
Beyond the FDA-approved schedule, additional cycles are sometimes given off-label or through trial protocols. Risks include cumulative radiation exposure to kidneys and bone marrow. Discussed case-by-case.
What if I miss a cycle by several weeks?
Short delays (1โ2 weeks) usually don't affect outcomes. Longer delays may. The team will work with you to reschedule. If you have to delay several weeks repeatedly, the team will consider whether continuing the course is right.
What does 'completing the course' mean for outcomes?
Patients who complete the full course generally do best, but the relationship isn't absolute. Some patients with partial courses still have excellent long-term outcomes. The opposite is also true. Cycle count is one factor among many.
